Well, what I've been taught to do is to lean foreward with the leg I'm kicking with use, jump with that leg while bringinig the opposite knee up while turning your upper body, then you swing your kicking leg around.
I think I described that right

The 360 roundhouse/ crescent kick or better known as the "tornado kick". (http://www.geocities.com/ox_pasture/T540.html)
From your right foot back, left foot forward, (your left foot fighting stance kk) and your body facing to the side, or right open stance.... take a full step forward with your right foot. Pivot on your right foot only and spin counter-clockwise with your left foot off the ground.
Then quickly jump off your right foot, kick out to the front with a crescent/roundhouse kick and continue to spin around counter-clockwise then land on the kicking foot and continue to spin in the same direction to complete the kick.
These tips have helped me out!
Use your arms to help you swing you body around quickly, building torque.
Tuck in your non-kicking leg!

Yeah that can happen, I'm not sure exactly why, maybe not bringing up high enough, or your spin is to much in the hips......I'm not sure it happens to me sometimes too

isn't all your power supposed to come from the hips though?

Break the kick apart ... as the link "tornado kick" that I posted above suggests. Its a difficult kick and takes alot of practice.
Maybe flyingblackbelt can provide some of his personal tips... to see him perform a tornado kick is an awesome sight!
