Looking for a dissertation

I'm looking for a copy of Kirstin Pauka's dissertation on Silek. It doesn't seem to be in any of the usual online places. And since it was submitted on CD-ROM instead of in print the dissertation reprinting services don't have it.

Does anyone know where to get a copy?
